80% of the new scooter FDA Serious
Injury reports are from tip overs and falls. Many of these new scooter
users could be placed in unnecessary, increased risk situations because
of the way the regulatory process and scooters work (or do not work)!
There is the presumption that “FDA Approved” products must be safe. In
fact, it is a violation of FDA Regulations to claim your 510-K product
is “FDA approved”.
A 510-K is a
Class II Medical Device premarket notification
process in which the FDA reviews the manufactures
submitted paperwork for clearance to sell the product.
The FDA does not test products for
510-K’s. The FDA reviews 510-K premarket paperwork voluntarily provided
by the manufacturer/distributor. The FDA relies on the honesty of the
applicant to do the required RESNA tests and accurately report the
results.
As we have previously reported, we are very concerned about the safety
of super short, narrow, lightweight scooters for everyday indoor usage.
A major U.S. manufacturer’s owner’s manuals warned these scooters are so
unstable that driving over a 1/2” indoor threshold or hitting an object
in reverse on a flat surface could cause the scooter to completely tip
over.

USERS SAY
BARIATRIC CENTER DRIVE CHAIRS DANGEROUS
|

In the past we
have explained why center drive 6 wheel spring suspension
power chairs are inherently the most unstable power chairs.
Because of this they are inappropriate for bariatric users.
The following actual user comments from a large
manufacturer’s web site provide accurate descriptions of
their problems.
|
|
“I have a
________. It has a number of design flaws, a couple serious
enough to make the chair dangerous”. “First, the chair had
to be designed with front anti-tip wheels because it lurches
forward when coming to a stop”. “My chair also rocks like a
rocking chair when it stands still”. “If I lean forward to
type on my PC, my chair tips forward”. “I can’t tell you how
many times I’ve almost slid off my chair because I had to
come to a sudden stop at the grocery store because someone
comes out of an aisle without looking first:. “And I have
fallen twice”. “And raising and lowering myself on my van’s
lift is a nightmare because my chair is in full forward tip
mode the whole time”. Another user’s comments about the same
model, “My ________ almost tipped over the other day because
it leaned forward too far and after only one year, one of
the front anti-tip wheel shocks broke”. (The chair uses a
spring front anti-tip system.) The inherent teeter-totter
problems of center drive chairs are greatly exacerbated with
a bariatric user’s super high center of gravity.

Our Boss uses a
midi 3/4 design with drive wheel between front and center
drive. This design has two advantages. First, the design
greatly reduces the inherent instability of center wheel
drive. The only time our adjustable rigid anti-tips are used
is when stopping quickly on a steep slope or using our
exclusive step off footrest chair access.
|

The 3/4 midi
drive design provides a super short turning radius very
close to the center drive radius. It is considerably better
than front drive and vastly superior to rear wheel drive
bariatric chairs.

Customer says
Rear Wheel Drive Can’t be Used Indoors, and it’s only good
for a doorstop!
|

Rear wheel drive bariatric chairs
large size and poor turning radius make them unusable for
indoor use. One customer actually wrote these comments on a
dealer web site in
frustration after the purchase of a rear wheel drive chair. Strengths - “Makes a good door stop”. “A bad design that’s time has long since outgrown its usefulness”. We continue to see manufacturers marketing rear wheel drive bariatric chairs even though the turning radius makes them unusable inside the average house. These manufactures hide this problem by not listing the terrible turning radius in their sales literature. The manufacturer of the chair the user comments are about goes even further to hide the design’s severe limitations. They don’t even reveal the huge turning radius in the Owner’s Manual. |

The manufacturer
must tell you the PWC turning radius so you can determine if
the chair can be used inside the house. Otherwise you could
be stuck with an unusable chair.
|
|
HOW TO
TRANSPORT BARIATRIC PATIENTS
|
|
From recent
discussions and watching automotive shows on TV, I believe
there is great misunderstanding about transporting a person
in a wheelchair in vehicles. In particular there are major
issues in transporting Bariatric persons. Let’s start by
discussing the enormous forces involved. In a 30 m.p.h.
crash you can see forces of 20 Gs (20 times the pull of
gravity). A 350 lb. chair with a 650 lb. rider equals 1,000
lb. at 1 G and 20,000 lb. or 10 tons at 20 Gs. To put that
in perspective, a Peterbilt Semi-tractor with crew cab
option only weighs 18,000 lb. or 9 tons. So if a van hits a
wall at only 30 m.p.h., the chair & rider are the equivalent
of a Peterbilt tractor + 2,000 lb.. flying into the back of
someone’s seat and out the front of the van. If a van stops
quickly by just braking from 60 m.p.h. the forces involved
are .5 to .75 G. We rate our 650 lb. Boss for a 7 degree
incline. For safety each 650 lb. Boss brake is tested to the
equivalent of a 15 degree incline fully loaded. But the
braking forces involved in holding a Boss with a 650 lb.
person when a van stops quickly can reach the equivalent of
a 30 degree ramp or 4.2 times the incline rated holding
power of the brake. There is no scooter or power wheelchair
brake I am aware of that can hold at this force level
repeatedly. Even if the brake did hold at .75 G, we still
are not talking about what would happen in a simple 5 or 10
m.p.h. parking lot impact. We need to secure the chair and
the rider.

A few years ago I visited a dealer who was doing a full package van conversion - power wheelchair platform lift, driving controls and bolt securement system. They had installed the bolt system in the floor and on the powerchair but I was surprised to see they were using the van’s seat belt system to restrain the occupant. When I returned home I called the Bolt System manufacturer and asked if it was acceptable to use their system with the user in the chair. They explained to me that their testing was for securing the wheelchair only. They do not take any responsibility for securement and safety of the rider if they are in the chair. Here is why that makes good sense. Let’s assume that the chair and rider are properly positioned in a safe zone so the air bag that explodes out at 200 m.p.h. in a crash doesn’t injure the rider. Remember the kids that died in improperly positioned car seats. Now in a 30 m.p.h. crash a 150 lb. rider stretches the belts like a string of a bow with the force of 3,000 lb. The belts release their energy and throw the rider back into the back and seat with great force. That back must be strong and tall enough to catch the person’s back and head and not collapse. With all the seating and back combinations therapists may use it is not possible to test them all in combination. The bottom line is the bolt system is only tested for securing the chair. |

So how can we transport a
Bariatric user and chair. First, you must have the
Bariatric person transfer to seating provided by the van
manufacturer. There is not a testing standard for a person
that is greater or equal to 450 lb. The person in a van seat
will be several times safer than in the wheelchair. Why, if
for no other reason than the securement points are not
designed to handle the Peterbilt combination of a 350 lb.
chair and a 650 lb. rider. Even with four rear hold down
points, by splitting up the total mass of the rider and
chair gives the user a much better survival chance. Next,
secure the chair separately. We will supply optional chair
tie down points on any new Boss chair when requested at the
time of sale. There must be two sets of rear floor
attachment straps to secure the chair to the floor. In
testing we did in 1988 with standard size chairs and riders
the first component that failed was the rear floor strap
attachment points. These comments are only suggestions. You
must follow the requirements of each of the component
manufacturers in the chair, seating system and van restraint
system.

wheelchairs are cheaper to build but will cost more in the
long run.

We have used moderately priced dual
modular component controllers exclusively for 4 years. While
they are more expensive than the “built for cheapest initial
cost integrals”, modular controllers are vastly superior in
performance, safety and total cost.
Joystick module. Integrals must compromise combining controller and power function in one box and do both functions poorly when compared to modules. Modular controllers are much less costly and easier to maintain over the life of the chair. Minor component repairs that are quick and inexpensive to repair on a module can require you to replace the entire $1,000.00 retail integral controller. If you damage the modular control circuit line, between the joystick and power module, it is a simple quick plug in of an inexpensive cable with no programming. With the joystick module the worst case is replacing the much less expensive joystick module. No programming required. In many cases the joystick throttle is easily replaced as a component and does not require replacing the joystick module. No programming required. Separate power modules, because they are an uncompromised design, have much bigger heat sinks protecting the power components much better and providing longer life and reliability. |

On integral controllers if you damage
the power control cable badly, in many cases you will have
to replace the entire $1,000.00 controller. If you run the
joystick throttle into/under a table (which is easy to do)
damaging it, you could have to replace the entire integral
controller. Integral controllers overheat much more quickly
doing the same load of work, significantly reducing
controller life.
Replacing an integral requires much higher labor costs. The product knowledge and time required to reprogram, remove and replace wiring from the battery to the controller and motor is much greater than simply plugging a control cable into the two modules. Modular controllers are much more
efficient and powerful. Inefficient integrals waste power because high current must travel 8 or more feet from the battery up to the arm mounted integral controller and back down to the motor. Because of wasting more power per trip from the heating of wires, batteries can get deeply discharged sooner. Deep cycling batteries dramatically reduces battery life. This inefficiency also means less use time per day and less range for heavy users. Uncompromised Power Module. Separate power modules have much bigger heat sinks (more metal to absorb heat) which makes the chair controller run much cooler and longer. The separate power modules provide much greater power efficiently. We use power modules providing massive 90 amp peak power per channel, 180 amp total in a moderately priced modular controller. With uncompromised power modules we do not see the FDA incidents other manufacturers are reporting of controllers rolling back or dying, leaving users stranded on hills or in the middle of streets. Even if the power module gets hot it is not next to the user’s arm or leg so it can’t burn them like integrals do. The integral compromised design has much smaller heat sinks because of limited space. Small heat sinks cause the metal case to get very hot. One manufacturer reported to the FDA several users burning their legs. One user required plastic surgery with skin grafts to repair third degree burns. Manufacturers have tried putting insulation pads on the inside of integral controllers to protect users from hot surfaces but this insulation caused the controller to over heat even faster, causing thermal roll-back and stranding more users. The modular controller design is inherently less prone to EMI malfunctions. Because high power lines are not touching and wrapped inside the same cable as the sensitive micro amp control lines, the electromagnetic waves created from the high power lines are not transmitted into the control cable causing erratic performance. When comparing power wheelchairs the controller is the first and most important quality feature to consider. |
